Registration with medical institutions

In accordance with the Law "On the fundamentals of protecting the health of citizens in the Russian Federation", every patient has the right to choose a medical organization.

No more than once a year, you can register with an outpatient clinic (except in case of changing the place of permanent or temporary residence).

HOW TO REGISTER WITH AN OUTPATIENT CLINIC?

Option 1. On-line

  • Application form can be submitted electronically in "Services " section on mos.ru
  • It takes 3 days from submitting the application to registration to accessing online appointments with the specialists of the chosen medical institution.
  • You can register only on your own behalf.
  • The service is available to users over 18 years of age with a valid Moscow CMI certificate.

Option 2. In person

  • The patient or his representative must personally submit the application for registration to the selected outpatient clinic.
  • The outpatient clinic that reviewed the application checks the provided information.
  • After completing the check the outpatient clinic notifies the patient of successful registration.

SHOULD I CANCEL MY REGISTRATION WITH THE PREVIOUS OUTPATIENT CLINIC?

NO. In case the patient changes the outpatient clinic, it is not necessary to cancel the registration with the previous one. The chosen outpatient facility will request to transfer all the necessary medical records on its own.

WHAT DOCUMENTS DO I NEED TO REGISTER?

When submitting an application, you must provide originals of the following documents:

For Russian citizens aged 14 and over:

  • Russian passport or temporary identity card, issued for the period of drawing-up of the passport;
  • Compulsory medical insurance certificate.

For children (up to 14 years) who are citizens of the Russian Federation:

  • Birth certificate;
  • Document certifying the identity of the legal representative of the child;
  • Compulsory medical insurance certificate.

For a representative of a citizen:

  • Documents certifying the representative's identity and authority.

In case of change of residence:

  • Document confirming the change of residence.

Compulsory medical insurance certificate

Compulsory medical insurance certificate (CMI certificate) is a document certifying the right of the insured person to receive free medical care throughout the Russian Federation within the basic program of compulsory medical insurance, in accordance with para. 1 sec. 45 of the Federal Law dated 29.11.2010 No. 326-FZ "On compulsory medical insurance in the Russian Federation".
